Clarity Community Connections
Our Purpose
Our purpose is to... provide case management services, mentorship, and education to high-risk individuals in need of resources, information, and support but do not meet criteria for case management through government agencies. We are currently focusing on improving two populations at risk in New Hampshire: women ages 18 and older who are starting over after incarceration or substance use disorder treatment; and high school and college graduates ages 18 or older who are struggling with financial, health, job, social, legal, or housing challenges. We believe there needs to be a safety net for people who do not qualify for Medicaid, Temporary Assistance for Needy Families (TANF), or Supplemental Nutrition Assistance Program (SNAP) benefits, but who do not have the resources to obtain medical care, substance use disorder treatment, or legal support. We also believe there should be support for individuals who meet criteria to receive assistance but lack the understanding of how to navigate the paperwork or even know where to start looking for help. We believe that, given the right circumstances, all people can thrive and be valuable members of their community. We understand that given the right circumstance, all people can thrive and be valuable members of their community. We help provide clarity so that individuals can make connections in the community and be their best selves.
